Is the Autotronic gearbox on the first Mercedes B-Class reliable?

It is the most delicate of the B-Class automatics. Ask for proof that its oil was changed, and test pulling away and climbing for slipping or a droning noise.

What should I check on a Mercedes B-Class 7G-DCT?

Test it cold in stop-start driving and when parking: jerks, hesitation or a clunk when engaging can mean the clutches or the mechatronic unit need work. Its oil and filter should have been changed on schedule.

Is an automatic B-Class good for city driving in Italy?

Yes, the automatic makes stop-start traffic easier, and a petrol or plug-in hybrid version suits short urban trips better than a diesel, whose particulate filter needs longer runs to regenerate.
